Harm Reduction Addiction Homelessness Homeless Vulnerable Populations Public Health

About

Dr. S. Monty Ghosh, MD, MPH, FRCPC, is an Internist, Disaster Medicine physician, and Addiction Medicine Specialist, and Associate Professor at the University of Alberta. He practices Internal Medicine at the University of Alberta Hospital in Edmonton and provides Addiction Medicine services at Foothills Medical Centre and Rockyview General Hospital in Calgary. He holds dual academic appointments at the University of Alberta and the University of Calgary.

Dr. Ghosh’s clinical and academic work focuses on addiction medicine, overdose prevention, harm reduction, disaster medicine, and health systems innovation.

He has addiction certification through the International Society of Addiction Medicine and holds a Master of Public Health from Harvard University, where he was selected as Commencement Speaker. His training integrates clinical medicine, public health, and systems-level policy development.

Clinical Leadership and Community Engagement

Dr. Ghosh works extensively with community-based not-for-profit organizations across Alberta to support structurally marginalized populations, including individuals experiencing homelessness, substance use disorders, mental health concerns, and those involved in the correctional system.

He previously provided community internal medicine at The Alex Community Health Centre and the Calgary Drop-In Centre, a shelter serving individuals experiencing homelessness. His work has included developing and implementing innovative, equity-focused programs that improve access to addiction treatment, harm reduction services, and integrated medical care.

He previously served as the co-founder and lead of Alberta Health Services’ Rapid Access Addiction Medicine (RAAM) program in Calgary and acute care addictions programming at several hospital sites. In this role, he oversaw clinical programming that supports individuals using alcohol, opioids, methamphetamines, cannabis, and individuals experiencing behavioural addictions such as gambling, sex, and video-gaming. The RAAM model provides rapid, low-barrier, evidence-based addiction care integrated within Alberta’s health system.

Research

Research, Policy, and Systems Impact:

Dr. Ghosh is heavily involved in provincial policy development in Alberta and contributes to national substance use strategy through research and implementation science.

He is the Nominated Principal Applicant or Principal Investigator on major competitive grants funded by:

  • Health Canada

  • Alberta Innovates

  • Alberta Health

  • Canadian Institutes of Health Research (CIHR)

He has secured more than $38 million in research funding as PI, Co-PI, or collaborator.

Dr. Ghosh leads a large, interdisciplinary research program focused on addiction, overdose prevention, harm reduction, and population-level substance use surveillance. His research integrates clinical medicine, implementation science, digital health innovation, and public policy.

Research Program: Addiction Medicine, Overdose Prevention & Harm Reduction Innovation

Overview

Dr. Ghosh’s research program focuses on developing, evaluating, and scaling evidence-based interventions to address substance use disorders and drug toxicity in Canada. The program combines clinical research, digital innovation, implementation science, epidemiology, and policy evaluation to improve outcomes for individuals and communities.

1. Overdose Prevention and Digital Harm Reduction

Dr. Ghosh is a national leader in virtual supervised consumption services and digital overdose response systems, including large-scale implementation and evaluation of remote monitoring models.

Research themes include:

  • Effectiveness of digital overdose response platforms

  • Mortality reduction strategies

  • Risk prediction and early warning systems

  • Implementation in rural and remote communities

  • Policy evaluation of supervised consumption models

This work supports scalable, technology-enabled harm reduction solutions that expand access to overdose prevention across Canada.

2. Wastewater-Based Surveillance & Drug Supply Monitoring

Dr. Ghosh leads initiatives using wastewater-based epidemiology (WBE) to monitor real-time changes in the illicit drug supply.

Applications include:

  • Early detection of fentanyl analogues and novel psychoactive substances

  • Community-level drug trend monitoring

  • Public health alert systems

This research strengthens surveillance capacity and supports rapid public health response to toxic drug events.

3. Health Systems and Policy Evaluation

Dr. Ghosh evaluates addiction-related policy frameworks, including:

  • Supervised consumption sites

  • Rapid Access Addiction Medicine (RAAM) models

  • In-hospital overdose prevention

  • Compassionate intervention and mandated treatment policy

  • Federal and provincial substance use strategy

His work uses mixed-methods evaluation, population health data, and implementation science frameworks to assess real-world effectiveness.

4. Structural Vulnerability and Equity-Focused Care

A core pillar of Dr. Ghosh’s research is improving health outcomes for structurally marginalized populations, including:

  • Individuals experiencing homelessness

  • Justice-involved populations

  • Rural and northern communities

  • Indigenous populations

  • People with concurrent HIV and hepatitis C

His program integrates peer-led research and community-engaged scholarship to ensure interventions are culturally responsive and equity-driven.

Dr. Ghosh has authored over 60 peer-reviewed publications in addiction medicine, public health, and health systems research.

Teaching

Dr. Ghosh’s teaching integrates clinical excellence, systems-level thinking, and equity-focused care. He emphasizes diagnostic rigor in Internal Medicine and Addiction Medicine while exposing learners to implementation science, overdose prevention strategy, and health policy design. His mentorship supports trainees in clinical practice, research development, and leadership within addiction medicine and public health.

Clinical Rigor and Practical Application

Dr. Ghosh emphasizes diagnostic precision, critical appraisal of evidence, and applied clinical reasoning. Trainees are taught not only to manage substance use disorders and complex internal medicine cases, but to understand the pathophysiology, pharmacology, and systems barriers that influence patient outcomes.

In addiction medicine teaching, this includes:

  • Evidence-based opioid agonist therapy

  • Management of alcohol, stimulant, and polysubstance use

  • Harm reduction frameworks in acute care

  • Behavioural addictions

  • Complex comorbidity (HIV, hepatitis C, homelessness)

He prioritizes bedside teaching, case-based learning, and interactive clinical discussions that mirror real-world complexity.

Systems Thinking and Policy Literacy

Given his work in overdose prevention research, digital harm reduction innovation, and provincial policy building, Dr. Ghosh integrates health systems education into clinical training.

Learners are exposed to:

  • Implementation science principles

  • Supervised consumption service evaluation

  • Wastewater-based drug surveillance

  • Addiction policy development in Alberta and Canada

  • Ethical considerations in mandated treatment and public health response

He encourages trainees to think beyond individual patient encounters and to understand addiction within structural, epidemiological, and policy frameworks.

Equity-Focused Innovation and Trauma-Informed Education

Equity principles and structural competency strongly inform Dr. Ghosh’s teaching. He emphasizes:

  • Trauma-informed care

  • Stigma reduction in addiction medicine

  • Working with marginalized and justice-involved populations

  • Culturally responsive care

  • Community-engaged models

He challenges learners to examine implicit bias and to develop comfort working with high-complexity and high-acuity populations.
